 A Volunteer’s Experiences and Musings about Management Failures and Fiascos at the Idaho Falls Zoo 

The main purpose of this report is to document the management failures that led to only a miracle preventing a mauling or death! and the subsequent failures in the accident follow-up.  A secondary purpose is to include management shortcomings in select other key areas. The public has a right to know how safe their zoo is and how well it is being managed. Due to website constraints the report is broken into three parts.  The first part is the main body of the report and  is structured as follows: 

• management failures that led up to the actual incident 
•  management failures post-accident
• other safety-related zoo failures 
• management failures with respect to the welfare of our animal friends 
• other non-safety-related management failures - personnel, leadership, project management and planning 
• failures of management overseers: AZA, USDA/APHIS and the City of Idaho Falls 
• ideas and suggestions of how to proceed 
• an epilogue provides some reflections. 
• Part 2 "Exhibits" is primarily photos 
• Part 3 "Appendices" is what I call the Who Knew What When and includes discussions with the Mayor and the Executive Director of the Zoo as well as three apex predator attacks, two fatal
